When a company grows, it goes quiet — and the quiet is invisible from the top.
Nobody decides it. You add people, layers and pressure, and at some point speaking up starts to cost more than staying quiet. Problems surface late. Ideas die in corridors. The people who carry your culture leave, and you find out why in the exit interview.
These questions don't produce a score.
So what's worth noticing isn't whether you can answer these. It's which one you have to think about.

When did someone last disagree with you in a meeting, with others watching?
Most people will disagree with you privately. Far fewer will do it in the room.
- Was it the same person it always is?
When did unpleasant news last reach you undiluted, from two or more layers down?
Information gets softer at every handover. What matters is whether it reached you at all.
- By design, or by accident?
The harder ones.
